Basic Requirements

1. Completed visa application form (dha-84) form 11
   • Must be filled out in print
   • Signed in black ink
2. Valid passport
   • No older than ten years and with a minimum validity of three (3) months beyond your planned stay in africa.
   • It must have at least two (2) blank pages in order to be able to affix the visa sticker. 
3. Two (2) copies- photocopy of passport bio-page
   • Two (2) copies- 4.5cm (high) x 3.5cm (wide) white background
     • Photo must be taken within the last three (3) months

4.  Vaccination certificate (yellow fever) applicable if:
   • Applicant will travel through or stay in yellow fever belt
     • African countries that require yellow fever certificate/ vaccine
5. Financial requirements
   • Bank statement and bank certificate
     • Must be a certified true copy with a wet signature for each page by bank manager
     • Three (3) months of bank account statements stamped by your bank.
     • Bank statement and bank certificate should not be older than seven (7) days from the time of application
6. Travel itinerary
   • Cover letter with travel itinerary must include the following details:
     • Cover letter sample
     • Applicant’s name
     • Passport number duration of visit address and contact details of hotel/s with authorized person’s name, designation and signature
     • Outlining the purpose and duration of visit
     • Letter must be certified/ notarized
     • Flight reservation/confirmation
     • Must include confirmed roundtrip ticket

   • Employed:
     • Certificate of employment
     • Payslips for the last 3 months

     • Photocopies of sec registration with attached present articles of incorporation and personal income

     • Mayor’s permit
     • DTI certificate
   • Latest income tax statement freelance worker:

     • Proof of enrollment

     • If you will be going on a holiday during summer break, submit a certification of reservation, or certification of enrolment to prove that you are enrolled for the next school year/semester.
     • If traveling during the school year, a certificate of leave of absence from school should also be submitted.

     • Birth certificate
