Self-Employed
- Corporation:
- Photocopies of SEC registration with attached Articles of Incorporation.
- Latest personal income tax statement.
- Single Proprietorship:
- Photocopy of business name registration (DTI Certificate).
- Latest income tax statement.
- Freelance Worker:
- Certificate of Registration.
- Certificate of Employment (if applicable).
- Payslips (optional).
Retiree
- Pension statement for the latest six (6) months.
- Proof of regular income generated by a property for the last six (6) months.
Student
- Proof of enrollment.
- School ID.
- Certification of enrollment or reservation for the next school year/semester if traveling during summer break.
- Certificate of leave of absence if traveling during the school year.
For Minors
- Birth Certificate.
- Completed Schengen-Swiss Visa Application Form signed by both parents.
- If one parent has full custody: DSWD Family Court Order.
- If both parents are alive: Certified copies of any government-issued IDs (e.g., UMID, Driver’s License, Pag-IBIG, PhilHealth, or Passport).
- If the child has a guardian: Certified copies of the guardian’s government-issued IDs.
- If the child is traveling alone: A notarized parental authorization to travel to Switzerland/Schengen signed by both parents/guardian(s).
If Sponsored by a Resident
- Proof of Relationship:
- Original birth certificates of both the visitor and host resident.
- Marriage certificates of the visitor and host resident.
- For boyfriends/girlfriends, fiancé(e), or friends: Photos together or other evidence of the relationship.
- PSA-issued Certificate of No Marriage (if widowed, provide PSA-issued death certificate of the late spouse).
- Photocopy of Host’s Passport:
- Bio page.
- If the host is not a resident, submit a photocopy of the residence permit as well.
- Letter of Invitation:
- A written and signed invitation from the host in Switzerland, including the full address, contact details, duration of the visit, who will bear travel and living costs, and whether financial support is guaranteed.
Additional Reminders
Submitting a visa application, including all the required documents, does not automatically mean that the visa will be issued. During the examination of an application, the Embassy may request additional documents and may call the applicant for an interview at the Swiss Embassy in Manila.
