Basic Requirements

1. Italy Visa Application Form
   • If minors (17 years old and below), both parents must sign the form
   • Put “N/A” for questions not applicable, don’t leave blanks
   • Provide 2 contact numbers on the form
2. Photocopy of the passport must provide the bio page, last page, pages with visas and stamps (entry and exit)
3. Photos Specification:
   • 2pcs 2×2” or passport size
   • Must NOT have been used for other visa application
   • Must be taken not older than 3 months
   • White background, with collar (hair must be behind the shoulders), both ears exposed, and mouth closed
4. Visa and VFS Fee. Cash payment only at the center. 
5. Travel Health Insurance
   • Minimum coverage of  EUR 30,000 (plus 15 days of allowance beyond travel period) and valid throughout the Schengen area for emergency hospitalization and repatriation expenses. You must also purchase only from a list of Italian Embassy-accredited insurance companies.
6. Purpose of Travel:
   • Copy of the travel itinerary like accommodation and flight.  For the flight itinerary, it is optional. 
   • Detailed day-to-day itinerary of the planned trip. 
7. Introduction letter, all applicants (exempt from Seafarers applying for Transit and Work Visa) must include in their application. It should be addressed to the Italian Embassy and indicate the following:
   • Occupation
   • Source of income,
   • Family background,
   • Travel history,
   • Travelling companion,
   • Purpose of travel,
   • Length of stay requested, and
   • Other important details.

*Please affix signature, contact number and e-mail address. For minors, must also have their own and signed by both parents. 

8. Financial Documents
   • Bank Documents must have: Bank Certificate with bank books / statements covering for the past 6 months.
   • If Employed, must have:
     • Certification of Employment and must have: Applicant’s Name, position, date hired, compensation and issued under company official letter.
     • Approved Leave of Absence.  
     • If Government Employees/Officials approved “Travel Authority”
     • ITR
9. Complete Business Documents needed:
   • Complete set of DTI/SEC Registration. Must include Article of Incorporation and General Information Sheet,
   • BIR Certificate of Registration,
   • BIR “ASK FOR RECEIPT” Plate,
   • BIR Permits and License,
   • Company’s Financial Statements, and
   • Company ITR.
10. If minors, Notarize the Affidavit of Support and Consent from the parent and include:
    • 2 valid id’s,
    • Complete Business Documents or COE and ITR,
    • Copy of the PSA Marriage Certificate, 
    • Bank Certificate supported by Bank Statements of the parents, and 
    • Must be signed by BOTH parents.
11. If homemaker, Affidavit of support from the spouse/partner. If the family or group will apply together and will share documents, provide a photocopy of the Financial Documents (of the sponsors) for each applicant.
12. Applicant’s
    • Birth Certificate, PSA issued. Photocopy and Original
    • Marriage Certificate,  PSA issued (if applicable).   Photocopy and Original

Additional Requirements

For Students

1. School ID
2. Enrollment Certificate with approved leave of absence
3. Copy of school calendar

For Applicants Through Representative

1. Must provide an Authorization Letter authorizing the person with copy of valid ID. 

For Retired Applicants

1. Proof of retirement (e.g., pension)

For Professionals

1. Copy of PRC card / IBP / any licensed card

For Religious Figures

1. Original and photocopy of Note Verbale issued by Apostolic Nunciature even applying for Tourism or for Personal purposes

Non-Appearance

1. Only for those with previous Schengen Visa issued in Manila.
2. Biometrics are not required for children 11 years of age and below. However, if the child turns 12 years of age before and during the travel, they are required to do biometrics upon submission.  

Additional Reminders

Submitting a visa application, including all the required documents, does not automatically mean that the visa will be issued. During the examination of an application, the Embassy may request additional documents and may call the applicant for an interview at the Embassy of Italy in Manila.
